The questions you have got wrong and have not yet put right.
Most wrong first, then whichever you have gone longest without seeing. Every row is the question’s own page, so you can read one without starting anything.
Not by being answered once. A question is off the list when you have carried it back up the ladder, and one wrong answer anywhere in the app puts it back to the bottom.
Three right answers, each on a different day and each on or after the day it is due, retire a question — 3 days, then 7, then a single check 21 days later. One wrong answer anywhere in the app puts it back to the next study day for this licence. Days off count; switching to another licence pauses this schedule.
The days are days you study, not days on a calendar. A fortnight away from the app moves nothing along, so nothing here is lost by stopping and nothing is gained by opening it and closing it again.
